Healthy Families Survey - Due by December 20 Hello Mountain Oaks Families,The window for the California Healthy Kids Survey is open.The California Healthy Kids Survey (CHKS) is an anonymous, confidential survey of school climate and safety, student wellness, and youth resiliency. It is administered to students at grades five, seven, nine, and eleven. It enables schools and communities to collect and analyze data regarding local youth health risks and behaviors, school connectedness, school climate, protective factors, and school violence. The CHKS is part of a comprehensive data-driven decision-making process on improving school climate and student learning environment for overall school improvements.California Education Code sections 51513 and 51938(c) specify that parent or guardian consent be granted before students are given questionnaires or surveys asking about personal beliefs or practices that include health behavior and risks. There are two kinds of parent or guardian consent: passive and active.Active Consent (for 5th graders) requires that a parent or legal guardian be notified in writing and gives written permission for the student to participate in the survey. Active consent is required of the fifth grade CHKS survey.Passive Consent (for 7th, 9th and 11th graders) requires that a parent or legal guardian be notified in writing about the survey and is given the opportunity to review the survey. Winter Craft Day December 13th from 10-3:00This wonderful event is open to all Mtn. Oaks families. It is a wonderful day full of crafts, fun, food and games. Crafts 10-12:00Potluck 12:30-1:00Cookie Exchange 1:00-1:30Volleyball Game 1:30 until we are worn outCrafts:We will have about 10 different crafts for kids to do. These make great gifts for family or friends or even to hang on your tree. Parents should plan on bringing a flat box of some sort to transport all of their wonderful treasures. Many of the crafts involve a lot of glue and therefore are transported best when they are not stacked on top of each other.Potluck:Everyone participating in the potluck should bring a dish to feed about 10 people. Please bring the following items if your Last Name starts with:A-J: Main DishK-S: Side DishT-Z: DessertCookie Exchange: This is a family favorite each year. Anyone interested in participating need to bring HOMEMADE Cookies or Desserts and 10 copies of the recipe to share with others. Participating families can sample the different items and take a copy of the recipe home to use later. Volleyball:Everyone is encouraged to stay and play volleyball. This is a lot of fun. Last year was amazing. We had parents, staff, high schooler and students as young as Kindergarten playing. It is good for kids to see that parents and staff can play games too.
